Answer a few short questions and we will get back to you within 2 working days. Please do not use this online form for urgent or emergency requests. The submitted forms will only be read during our opening hours. The forms will not be read on the weekends (Saturdays and Sundays) or on bank holidays. They will not be read Monday to Friday between the hours of 6pm to 9am. Please contact NHS Direct Wales on 0845 46 47 during out of hours or 999 if this is an emergency.

About our Triage Appointment System

We operate a hybrid triage system offering a mixture of telephone consultations, on the day emergency consultations with a doctor or pre-bookable (3 days in advance) GP or Nurse Practitioner appointments at a time convenient to you.

Booking Your Appointment

You can make an appointment with a doctor by phoning the surgery on 01792 863 103. The receptionist will take details of your request and this will be passed to the GP who will call you back to discuss. The GP will offer advice or arrange and appoinment if necessary. 

The GP will endeavour to ring you back within an hour of your initial call. 

All nurse appointments are bookable in advance. Please speak to the receptionist who will provide you with the earliest appointment.
