1. Routine GP Appointments:

• You can request a routine pre-bookable appointment up to 2-weeks in advance with a clinician by completing this form.

• We will review your request and contact you back with confirmation of when your appointment is booked for.

• We understand not all patient can use online services. As such, patients can request a routine pre-bookable appointment via phone by calling us on 01792 863103 – it is worth noting that same day urgent appointment requests are likely to be made by patients first thing in the morning and we therefore recommend, if your request is not urgent, that you call us after 10am to avoid waiting on the phone unnecessarily.

2. Urgent (same day) appointment requests:

Please call the practice on 01792 863103 from 8am onwards if you deem your request to be medically urgent. We will take details of your concern which in turn will be reviewed by the on call GP, with an appropriate appointment provided. Please note, we have limited capacity for on the day appointments and as such requests deemed not to be urgent may be offered a pre-bookable appointment on another day.

3. House Visits:

Please call the practice as soon as you are able from 8am and ideally before 12am if you require a house visit. Please note, the request will be reviewed by the on-call GP and you may receive a call back initially to review the request. House visits are restricted to patients who are physically unable to attend the practice. If you are able to attend, we ask that you avoid requesting an unnecessary house visit.
